Policy regarding non-participating insurance plans:

In the event that we do not participate with your particular insurance plan, you may still be seen by one of our doctors. Payment in full is expected at the time of service.

PAYMENT POLICY

We gladly accept personal checks, cash, MasterCard/Visa/Discover credit cards and debit cards.

If your check is returned by your bank for insufficient funds or for any other reason, a $30 service charge will be charged to your account. We will then ask you to pay for all future visits by cash or credit card.

We will file your insurance carrier as a courtesy for services provided to you in our office. You are required to present your insurance card at the time of the visit. Without a current insurance card, we will not be able to file your claims appropriately and you will be responsible for the payment of all charges. If your insurance coverage changes, please notify us at the time of your visit. We may or may not be able to refile claims and you would be responsible for payment. We suggest that you always carry your insurance card with you.

Your insurance company requires our office to collect co-payments, co-insurance, and deductibles at the time the service is rendered.

Self-pay patients are seen in our office. Payments for services are required at the time of the visit.

This office does not participate in any “third party billing”. In the event of an accident, etc., the patient is fully responsible for payment of all charges.

Our office performs many tests, services, and procedures. If you are concerned that your insurance may not cover certain services, please contact your insurance company prior to the date of service. If the service is not covered, you may be responsible for payment of the charges.

Laboratory tests are normally drawn here in our office. Some exceptions are those made by some insurance companies that require you to go to a particular laboratory. Laboratory Corporation of America performs most of our tests. There may be some tests performed that LabCorp would bill you for.

Policy regarding copying of medical records:

There is a 72-hour turnaround time after the medical records release has been signed.

It is our office policy to charge a fee for appointments missed, canceled or rescheduled without a 48-hour notice. It will be $10 for a lab appointment and $30 for an appointment with the doctor.

  • If you need a prescription refill, please have your pharmacy fax the request to 910.920.4224. Please allow a minimum of 48 hours for the refill to be processed. Do not wait until you are out of your medication as the doctor may not have access to your chart if it is a weekend or a holiday. It is important that requests are made during regular business hours.
  • Not all insurance policies cover annual physicals exam or the lab work associated with the exam. It will be your responsibility to contact your insurance company to verify coverage.
  • Not all insurance policies will cover procedures that are recommended by your doctor, for example mole removals. It will be your responsibility to contact your insurance company to verify coverage.
  • If your insurance company has required you to choose a primary care physician and you are coming to us as a new patient, it is extremely important that you call your insurance company and have the PCP changed to the doctor you are seeing. If not, the insurance company will deny the claim and the balance will be your responsibility.
  • If you do not have your insurance card with you at the time of your visit it may, not be possible for us to find the necessary billing information therefor you will be responsible for the entire amount at the time of your visit.
  • It is the patient’s responsibility to inform the front desk of any changes to your address, phone number, or insurance coverage. If your insurance coverage has changed and you fail to notify us of this change, this will result in us filing a claim with the incorrect insurance information and the patient may receive a bill as a result.
